GetLean helps patients in Singapore taper off Mounjaro and other GLP-1 medication through the GetLean GLP-1 Taper Plan, where clinically appropriate. Dr Quek plans the taper at your first 30-minute video consultation and steps the dose down at monthly reviews; in SURMOUNT-4, where tirzepatide was switched to placebo instead, weight rose 14.0% on average over the next 52 weeks (Aronne 2024). When the STEP 1 extension stopped semaglutide overnight, about two-thirds of the lost weight returned within a year (Wilding 2022); individual results vary.
GetLean Clinic is licensed by Singapore's Ministry of Health under the Healthcare Services Act 2020; everything happens over secure video and messaging, and, where a prescription is issued, first delivery is typically the next working day after your consultation.
If you are already taking Mounjaro or another GLP-1 medication with a different doctor, you can move to GetLean: tell us your medication and dose, bring recent records, and Dr Quek reviews your history by video. If you have never taken GLP-1 medication, you begin with the exit already planned. This page covers who GetLean helps, how moving from another clinic works step by step, and what a taper involves.
Taper: bringing a GLP-1 dose down in steps under a doctor, rather than stopping at once. Doctors also call it a step-down or reverse titration.
In Singapore, tirzepatide is registered as Mounjaro, and its registered indication includes weight management (Singapore NDF). Which medicine suits a particular person, if any does, is decided by the doctor who assesses them.
Who does GetLean help taper off GLP-1 medication?
Two kinds of patient: people already on treatment with another doctor, and people who have never taken GLP-1 medication.
Already on a GLP-1 with another doctor. GetLean welcomes you whether you plan to continue for now or to work toward coming off eventually. Dr Quek reviews your treatment with the aim of reaching your lowest effective dose and, where clinically appropriate, a planned exit.
Never taken GLP-1 medication. The programme is designed to end from day one. The taper is planned from the first consultation, and daily WhatsApp coaching builds the eating and training habits while the medication is working.
Can I switch my Mounjaro treatment to an online clinic?
Yes. GetLean Clinic is licensed by Singapore's Ministry of Health under the Healthcare Services Act 2020, for outpatient medical service delivered remotely, and people already taking Mounjaro with another doctor can move to it. How the online clinic works is on the online clinic page. Moving means a new assessment, not a transfer of your old prescription: a new doctor in Singapore assesses you and writes their own prescription (Therapeutic Products Regulations).
Moving to GetLean does not have to mean starting over: where it is clinically appropriate, your existing dose can typically be continued rather than restarted. That is Dr Quek's decision at the consultation. Nothing is prescribed or dispensed before he has assessed you, and a consultation does not guarantee that a prescription will be issued.
How does switching to GetLean work, step by step?
- Book your consult. We ask for personal details from Singpass and basic physical and medical information, including any medication you take now. Tell us which GLP-1 medication and dose it is.
- Gather your records. The name and dose of your current GLP-1 medication, recent records, the other medicines and supplements you take, your health history, your identification, and an honest account of the treatment so far, including any missed doses. Patients have a right to their medical information, so a medical summary or report from your current clinic is one way to bring records (SMC).
- Video consultation with Dr Quek. A first-time patient of a telemedicine licensee must be seen by real-time two-way video (OMS Regulations), and medicines should not be prescribed from a questionnaire alone (Circular 87/2024). Dr Quek verifies your identity before the consultation begins, then reviews your history. If he wants further checks at a physical clinic before prescribing, that is a normal step, not a rejection.
- Medication and delivery. Where a prescription is issued, the medication is delivered discreetly to your door, and first delivery is typically the next working day after your consultation.
- Your plan. A personalised nutrition and daily activity plan, and daily WhatsApp coaching. A plan assistant sends reminders, check-ins and progress tracking; it gives no medical, nutrition or dosing advice, and anything clinical goes to Dr Quek. Your care team is on WhatsApp from 9am to 9pm every day, Singapore time. Outside those hours an urgent symptom does not wait for a reply: go to an emergency department, or call 995 if it is an emergency.
- Monthly reviews. Dr Quek reviews how it is going every month. Dose changes are made at that check-in, towards the lowest effective dose.
The first video consultation has its own fee; after that, one monthly fee covers your consultations with Dr Quek, the medication and daily WhatsApp coaching, with no lock-in; the pricing page has the figures.
While the move is arranged, follow your current doctor's instructions and never stop or change a prescribed medicine on your own. If you are running low, see what to do if your clinic stops prescribing or you run out.

What does a taper involve?
A taper brings the dose down in steps, under your doctor, at a pace set for you, while the habits that replace the medication's effect are already in place. At GetLean, Dr Quek plans it from your first consultation and sets each step at the monthly review as your response comes in.
Why step down rather than stop at once?
The trials that tested stopping took the medication away: STEP 4 and SURMOUNT-4 switched people to placebo (Rubino 2021, Aronne 2024). In the STEP 1 extension, where the medication and the trial's lifestyle programme both stopped at week 68, about two-thirds of the lost weight returned within a year (Wilding 2022). Those are group averages, and individual results vary.
Staying on a lower dose is a different picture from stopping, and it is not the same as coming off. SURMOUNT-MAINTAIN tested staying on tirzepatide at a lower dose of 5 mg after 60 weeks on a higher dose, not coming off it: that group was still 16.6% below its starting weight a year later, and about 1 in 4 of them received rescue tirzepatide, which the trial allowed once more than half of the lost weight had returned (Horn 2026). It does not show that a taper to zero holds. Individual results vary: that is a trial result for a studied group, not a plan for any one person.
How does GetLean step the dose down?
Gradually, at the monthly reviews with Dr Quek, rather than stopping it overnight, as the STEP 1 extension did. The taper is planned from the first consultation and set as the response comes in, so each patient's schedule is set for them. Dose changes are made at the monthly check-in, with the lowest effective dose as the aim, and each one is Dr Quek's decision. Dr Quek builds the plan that takes you off the medication, where clinically appropriate, and your care team is on WhatsApp from 9am to 9pm every day, Singapore time, while the dose comes down. Outside those hours an urgent symptom does not wait for a reply: go to an emergency department, or call 995 if it is an emergency.
Does GetLean use microdosing to taper?
Yes. At GetLean, microdosing means taking the medicine below its labelled dose or schedule, and the GetLean GLP-1 Taper Plan gets you there in two ways, and often uses both: lower doses, where you come down to the dose you were on before and stay there until your hunger and cravings are at a level you can handle, and longer gaps between injections, where instead of one injection every 7 days, you might inject every 8, 9 or 10 days. The two can be combined: for example, going from Mounjaro 15 mg once a week to 10 mg every 10 days.
One safety rule: on higher doses (7.5, 10 or 15 mg), if you go more than 10 days without an injection, the taper restarts on a lower dose. Taking a high dose after a long break can cause stomach and gut side effects, including pancreatitis. Speak to us before your next dose. GLP-1 microdosing at GetLean, step by step sets out the whole plan, and every GetLean GLP-1 Taper Plan is set with Dr Quek: book a consultation to plan yours.
What replaces the medication's effect?
The habits. While the medication is working, the daily WhatsApp coaching builds the eating pattern and the training habit, with the aim that both are routine by the time the dose comes down. Photograph or describe a meal and the plan assistant estimates the calories and protein and shows what is left for the day.
Clinical-trial figures describe the populations studied, and individual results vary. Never change or stop a prescribed medicine on your own: that is a conversation with the doctor who prescribed it.
What happens after the taper?
Finishing the taper does not have to end the support. After the taper: daily coaching and a monthly review, no medication: S$88 a month, if you wish; what continuing includes, and what it costs, is on the pricing page. If the weight starts coming back, you can return to Dr Quek at any time: a restart begins with a review consultation, and whether to restart the medication is his decision at that review. There is no lock-in: you can cancel at any time.

It is a doctor-led GLP-1 programme designed to end: the taper is planned from the first consultation, Dr Quek reviews the dose every month towards the lowest effective dose, and daily WhatsApp coaching builds the habits that carry on after the medication.
Do I have to stop Mounjaro before I move to GetLean?
No. Never stop or change a prescribed medicine on your own: follow your current doctor's instructions while the move is arranged, and tell us which medication and dose you take when you book your consult. Will GetLean prescribe Mounjaro if I move over from another clinic?
Yes, where Dr Quek finds it clinically appropriate once he has assessed you by video. GetLean carries only GLP-1 medications approved by Singapore's Health Sciences Authority, such as Mounjaro. If you are prescribed treatment, Dr Quek plans the taper from your first consultation and sets each step at the monthly review. Nothing is prescribed or dispensed before he has assessed you, and a consultation does not guarantee that a prescription will be issued.
